The Oral Health Assessment Form (OHAF) is used to implement the DC Student Health Care Act of 1985 (DC Law 6-66), which mandates periodic dental examinations and submission of certificates of dental health for students attending the pre-kindergarten through grade 12 in public, public charter, private, and independent schools in the District of Columbia (§38-602).
